Pour in the remaining milk and the heavy cream, then set the pan over medium heat. Whisk frequently and bring the mixture to a gentle simmer, about 4 to 5 minutes; do not let it reach a rolling boil or it may scorch.

Reduce the heat to low and add the chopped semisweet chocolate. Whisk continuously until the chocolate is fully melted and the drink looks glossy and slightly thickened, 1 to 2 minutes.

Remove from the heat and stir in the vanilla extract. Taste and adjust sweetness if needed.
